Application for Variation of Service / Particulars of Hospitals

• Change of name or contact information of hospital, licensee, authorized representative of licensee, or Chief Medical Executive (CME)
• Change of authorized representative of licensee or CME
• Change in the membership of Medical Advisory Committee

• Change in the scale or scope of services as specified in the hospital licence
• Change in the physical location where the services specified in the annex of the hospital licence
• Material change to the premises of the hospital, including but not limited to –
  1. - Additions, alterations and/or improvement to the electrical installations for critical care areas, specialized ventilation systems, or medical gas pipeline systems
  2. - Change in non-movable walls of clinical or clinical supporting services e.g. renovation
  3. - Change in water treatment and distribution system for haemodialysis
• Extension of a hospital block or redevelopment of existing hospital block

1. Read the Code of Practice for Private Hospitals, guidance notes for the application, application form and all relevant forms and documents before application
2. Fill in the application form through
3. Duly sign the application form and the corresponding declaration forms (where applicable)
4. Prepare all required supporting documents
5. Submit all the document listed in (3) and (4) to the Department of Health through electronic or paper channel
6. Pay the application fee (if applicable) on or before the due date stated on the general demand note. Payment methods are specified on the general demand note

The validity period would be the same as specified in the existing licence
