Application for Renewal of Day Procedure Centre Licence

1. Licensee shall apply for renewal of day procedure centre licence not less than 6 weeks before the expiry of the licence
2. Read the application form and the relevant form before application
3. Fill in the application form through
4. Duly sign the application form and the corresponding declaration form
5. Submit all the documents listed in (4) to the Department of Health through electronic or paper channel
6. Pay the application fee on or before the due date stated on the general demand note. Payment methods are specified on the general demand note

The application fee for renewal of day procedure centre licence is HK$2,180

A renewed licence for a day procedure centre is valid for a period of not more than 3 years
